The PrEP Navigator will function as the primary contact for all of VFC's patients as well as providers and the general public for information regarding PrEP and HIV Prevention. They will create data parameters in our Electronic Health Records for effective in reach to the clinic population. They will independently monitor patients who are on PrEP, ensuring they follow up for additional medical care. As the main person of contact, they will work independently to provide education, training and support to clinic staff on an ongoing basis.

The PrEP Navigator will work with HIV testing and clinical staff to identify potential PrEP clients and those in need of PrEP navigation services. They will ensure HIV negative individuals who are interested in starting PrEP are connected to care and retain those already utilizing PrEP care. This person will assess benefits and insurance, and if needed, provide assistance to obtaining those. They will provide HIV prevention and PrEP adherence education and counseling. They will effectively collaborate with all individuals involved in PrEP. They will make referrals for external services needed to support PrEP care and follow up to ensure service delivery. They will provide ongoing follow up to ensure PrEP adherence. They will serve as a fully functioning member of our team, able to participate in daily activities and functions to maintain overall Common Ground operations.

Salary range: $29.64 - $39.13 Hourly

Qualifications

Required:

  • Knowledge of HIV/AIDS, community resources, health education methods, advocacy, harm reduction and general clinical knowledge.

  • Robust knowledge of PrEP/PEP.

  • Bilingual in English and Spanish.

  • Ability to create and maintain boundaries with clients

  • Ability to complete required program documentation.

  • Public speaking and leadership skills, and skill in managing multiple deadlines and demands.

  • Ability to work independently and as part of a group.

  • Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality.

  • Ability to work effectively with people of diverse cultural and educational backgrounds.

  • Skill in creating flyers and other program-promoting documents.

  • Ability to travel between sites (622 Rose Avenue and 604 Rose Avenue).
